ABSTRACT

Objective:To systematically evaluate the effects of protein restriction, low-sodium diet and alkaline diet on renal outcomes, the rate of change in estimated glomerular filtration rate (eGFR) and all-cause mortality in chronic kidney disease (CKD) patients.Methods:Three main databases, Ovid, EMBASE and the Cochrane Library database, were searched for randomized controlled trials about the effects of protein restriction, alkaline diet, low-sodium diet in chronic kidney disease. The primary outcome was renal composite endpoint events, the annual rate of change in eGFR and all-cause mortality. Renal composite endpoint events was defined as >25% or 50% decrease from baseline in eGFR, doubling of serum creatinine, or the development of end-stage renal diseaseas during follow-up.The studies were selected according to inclusion and exclusion criteria and assessed for quality using Jadad Scale. Two investigators were chosen to search, extract and evaluate the data independently. Software Stata 16.0 and RevMan 5.4 were used for meta-analysis.Results:A total of 34 studies with 5 589 participants were included. Protein restriction ( RR = 0.78, 95% CI: 0.64 to 0.96, P < 0.001), alkaline diet ( RR = 0.64, 95% CI: 0.43 to 0.98, P < 0.001) and low-sodium diet ( RR = 0.45, 95% CI: 0.28 to 0.73, P < 0.01) reduced the risk of renal composite outcomes. Protein restriction ( MD = 1.85, 95% CI: 0.77 to 2.93, P = 0.001), alkaline diet ( MD = 1.45, 95% CI: 0.53 to 2.37, P < 0.001) and low-sodium diet ( MD = 1.84, 95% CI: 1.06 to 2.63, P < 0.001) also decreased the rate of delince in eGFR. But these dietary patterns did not show a clear beneficial effect for all-cause mortality ( RR = 1.15, 95% CI: 0.76 to 1.73, P = 0.167 for protein restriction, RR = 0.96, 95% CI: 0.31 to 3.02, P = 0.546 for alkaline diet and RR = 0.82, 95% CI: 0.48 to 1.40, P = 0.057 for low-sodium diet). Conclusion:The three dietary interventions may decline the rate of renal function exacerbation and decrease the risk of unfavourable renal outcomes in CKD patients, while have no clear beneficial effect on all-cause mortality.

ABSTRACT

Objective@#To understand the association between bullying and anxiety symptoms of boarding middle school students, and to provide a scientific basis for preventing bullying and promoting mental health of boarding middle school students.@*Methods@#By using stratified cluster random sampling method, 2 823 students were selected from 10 schools in Anyang, Henan Province. Questionnaire survey using self-designed bullying items and Mental Health Scale of Chinese Middle School Students was administered.@*Results@#The overall prevalence of school bullying was 37.9%, with boarding school students(38.9%) being higher than non-boarding school students (37.1%). The anxiety symptom reporting rate of boarding students (42.9%) was higher than that of non-boarding students (27.3%). Binary Logistic regression analysis showed that, compared with non-boarding students, bully victim and bully perpetuator/victim students were more likely to have anxiety symptoms (OR=2.30, 6.04,1.94, 4.23) (P<0.05).@*Conclusion@#There is a correlation between different roles of campus bullying and anxiety symptoms among boarding and non-boarding school students. Boarding school students have a higher risk of anxiety symptoms, especially among those with both experiences of bully perpetuator/victim.

ABSTRACT

Objective To summarize experience and characteristics of the perioperative anesthetic management of patients with active infective endocarditis (AIE) undergoing cardiac valve surgery under cardiopulmonary bypass (CPB).Methods A total of 117 patients of both sexes,aged 18-70 yr,of American Society of Anesthesiologists physical status Ⅱ-Ⅳ,undergoing cardiac valve surgery under CPB from January 2014 to June 2015 in Peking Union Medical College Hospital,were enrolled in the study and divided into AIE group (n =57) and non-AIE group (n =60).Anesthesia was induced by Ⅳ injection of midazolam 1-2 mg,etomidate 0.15-0.30 mg/kg,sufentanil 0.3-0.5 μg/kg and rocuronium 0.6-1.0 mg/kg.After orotracheal intubation,the patients underwent high-frequency volume-controlled ventilation with low tidal volume,and positive end-expiratory pressure was increased appropriately.Esophageal echocardiography was used for evaluation.Anesthesia was maintained by inhalation of 1.0%-1.5% sevoflurane,intermittent Ⅳ boluses of sufentanil 0.2-0.3 μg/kg and Ⅳ infusion of piperium bromide 2 mg/h,and the bispectral index value was maintained between 40-60 during surgeDy.Sensitive antibiotics were given during surgery according to the results of preoperative blood culture in AIE group.The following data were collected:baseline patient characteristics,preoperative blood routine,erythrocyte sedimentation rate,concentrations of hypersensitive c-reactive protein and amino-terminal pro-brain natriuretic peptide,operation time,CPB time,intraoperative consumption of vasoactive drugs,fluid intake and output,use of antibiotics,length of postoperative intensive care unit stay,extubation time,new invasive operation (intra-aortic balloon counterpulsation was performed),time for use of vasoactive drugs,postoperative 24 h drainage,length of hosptial stay,serious complications (postoperative bleeding,pericardial tamponade,severe arrhythmia,heart failure,acute lung injury,renal failure and other organ function failure,etc.) and development of death.Results Compared with non-AIE group,age,body mass index and preoperative Hb,Hct,systolic blood pressure and diastolic blood pressure were significantly decreased,the rate of preoperative hypertension,neutrophil count,erythrocyte sedimentation rate,concentrations of hypersensitive c-reactive protein and amino-terminal pro-brain natriuretic peptide,body temperature and heart rate were increased,the intraoperative volume of allogeneic blood transfused and consumption of norepinephrine,phenylephrine and ephedrine after CPB were increased,the time for postoperative use of vasoactive drugs,extubation time and length of postoperative intensive care unit stay were shortened,and the length of hospital stay was prolonged in AIE group (P<0.05).Conclusion For the patients with AIE undergoing cardiac valve surgery under CPB,comprehensive evaluation should be performed and heart failure be corrected actively before surgery;esophageal echocardiography should be used for evaluation,the application of vasoactive drugs could be increased appropriately and anesthetists should pay attention to respiratory management during surgery;cardiotonic and anti-shock therapy should be maintained,arrhythmia prevented and anti-infective therapy strengthened after surgery.

ABSTRACT

AIM:To explore the therapeutic effect of Buyang-Huanwu decoction (BYHWD) on experimental au-toimmune encephalomyelitis ( EAE) and its immunoregulatory effect on monocyte-macrophages .METHODS: Chronic EAE was induced by myelin oligodendrocyte glycoprotein peptide fragment 35-55 ( MOG35-55 ) in the female C57BL/6 mice, which were randomly divided into saline group and BYHWD group .On day 3 after immunization , the mice in BYHWD group were orally administrated with BYHWD , while normal saline was given to the control mice .The clinical score and body mass were recorded every other day .At day 17 after immunization , the mice were sacrificed and spinal cords were obtained for HE staining and myelin staining .The M1 and M2 macrophage phenotypes of splenic cells were detected by flow cytometry and immunofluorescence staining .The protein expression of iNOS , TNF-α, arginase and IL-10 in the spinal cord macro-phages was determined by Western blotting .RESULTS:BYHWD delayed the onset of EAE , reduced the clinical scores of EAE, inhibited the inflammatory cell infiltration and demyelination in the spinal cord , and promoted the conversion of M 1 macrophages into M2 phenotype in the spinal cord and spleen .CONCLUSION:BYHWD intervention attenuates the be-havioral and pathological changes in the EAE mice , and its mechanism may be related to the macrophage conversion .

ABSTRACT

OBJECTIVE@#To examine the expression of microRNA-140 (miR-140) in osteoarthritis (OA) graded by X-ray diagnosis criteria of Kellgren and Lawrence and to provide a theoretical basis for further studing the role of miR-140 in OA development.@*METHODS@#Knee synovial fluid specimens were collected in 40 OA patients (the OA group), the OA group was divided into 4 subgroups according to X-ray diagnosis criteria of Kellgren and Lawrence: gradeIsubgroup (n=10), gradeII subgroup (n=10), gradeIII subgroup (n=10), and grade IV subgroup(n=10). Twenty patients served as a control group (the control group), including meniscus injury, cruciate ligament injury, trauma patients. The expression level of miR-140 in the knee synovial fluid was detected by real-time fluorescent quantitative polymerase chain reaction (stem-loop RT-qPCR, TaqMan). U6 snRNA was used as the reference control, and Ct value was calculated.@*RESULTS@#MiR-140 was detected in all knee synovial fluid specimens. The expression level of miR-140 in the OA group was much lower than that in the control groupb (P<0.05). There was significant difference in the expression level of miR-140 between the grade IV subgroup and the gradeI, gradeII and gradeIII subgroups (all P<0.05). The expression levels of miR-140 was negatively related to the severity of OA (r=-0.917, P<0.05).@*CONCLUSION@#MiR-140 in human knee synovial fluid can be detected conveniently by real-time PCR. The expression level of miR-140 decreases in the knee synovial fluid in OA patients, and is negatively related to the severity of OA. This may be related to the occurrence and development of OA, and can provide early potential diagnostic molecular target.

ABSTRACT

A new type of gastric electrical stimulator (GES) was introduced. After the stimulator was implanted in beagle dogs, its stimulating effects and the pathological changes at the implant site were observed to study the safety and efficacy of stimulator as well as the tissue compatibility of the materials used. The results showed that, this type of stimulator was safe and capable of inhibiting food intake of the dogs, and that the materials used had good tissue compatibility.

ABSTRACT

Objective To investigate the therapeutic effects of atorvastatin on pulmonary fibrosis of rats induced by Bleomycin (BLM). Methods Fourty-two female Wistar rats were randomly divided into 7 groups: the control group(group C) , the model group which was furtherly divided into group 2-week (M2) , 4-week (M4) , 6-week (M6) , and atorvastatin-treatment group which was furtherly divided into group 2-week (A2) , 4-week (A4) , 6-week(A6). Group M and A were induced to pulmonary fibrosis by the method of BLM endotracheal injection, while group C was injected with saline. On 2nd day, group A were given orally atorvastatin by 10 mg/kg · d. Rats were seperately killed on 2nd, 4th and 6th week. After intratracheal injection of BLM, alveolitis and pulmonary fi-brosis were evaluated by pathology, hydroxyproline concentration and PaO_2. Results The lung coefficient of group M2, M4, A2 and A4 was significantly higher than that of group C. The degree of pulmonary fibrosis in group A4 and A6 was improved as compared with group M4, M6. Alveolitis and pulmonary fibrosis in group A were improved compared with those in group M. Hydroxyproline concentration in group A and M were significantly higher than that in group C. While A4 was lower than M4 (P <0. 01). There was a decrease in PaO_2 after exposure to BLM, espe-cially in group M2 (P <0. 01). Improvement in PaO_2 was documented in groups A, compared with M. Conclusion Atorvastatin has centtain efficacy in treatment of pulmonary fibrosis of rats. The effect was significant on 6th week after treatment, which suggest that the effect is correlated positively to the carly treatment.
